Physical Incentive Program

We are more than halfway through the year and there are still many employees and spouses who have not gotten their annual physicals.

Annual check-ups are one of the best ways to maintain good health. That is why Star asks all employees and their spouses to take the time to see their family physician each year for a physical evaluation to qualify for Star’s wellness incentives. New employees and their spouses are encouraged to attain a physical to receive wellness incentives once they are benefit eligible. Employees receive a $1,000 incentive and spouses receive a $500 incentive. When you are enrolled in medical coverage, the annual physical is free and Star will pay our employees for up to two hours of time to receive the physical!

While our plan DOES allow for one physical per calendar year, your physician may ask that you wait 365 days between your annual physicals.

As a reminder, physicals must be completed by your primary care physician. Physicals completed at a drug store clinic, urgent care center, or company health fair will NOT be eligible to receive the $1,000/$500 wellness incentive.

Here is information regarding the physical payments:

  • Employee/spouse MUST submit a completed form (which can be found on Star’s intranet) to the Benefits department to receive the incentive.
  • Payment for the physical will be issued via ADP payroll.
  • Payment will be issued on the 3rd Friday of each month when the completed form is received by the last day of the previous month.
  • Employee must be actively employed on the date that the incentive is paid.
  • Incentives are for benefits-eligible employees only.
  • ALL wellness incentives are taxable per IRS regulations.
  • Employees with garnishments and/or child support payments could have the payment delayed due to federal regulations (lump sum).
  • Incentives are payable only once per employee/spouse per year.
  • Incentives apply to this year and changes may occur in future years.
